Your First Visit

Preparation for your first visit is essential in helping us to see you and all patients in a timely manner.

Please bring the following with you to your appointment:

• Your insurance card (check here to see our current list of insurance plans).

• A valid, government issued photo id (drivers license, passport, etc).

• For patients coming in for Nutritional Medicine, please bring in or fax your previous medical records, reports, and laboratories.

• The New Patient History Packet. Please complete and sign all forms.
